Something many take for granted is how easy it is to have access to the important documents required for us to get essential services. For example, your birth certificate. You often need it when registering for a new school, getting a driver’s license, when you get married and other big moments in life.

A frequent fact for foster care youth is that they often have no clue where their birth certificate is, much less how to get ahold of it. This makes it that much harder to achieve those previously mentioned big moments. Luckily, you get a copy of your birth certificate from the NY State Dept. of Health.

Below you’ll find the mail-in applications for receiving a copy of your birth certificate. BE SURE you use the correct version: there’s one application for youth born in New York City (Brooklyn, The Bronx, Manhattan, Queens or Staten Island) and another application for youth born elsewhere in New York State.
